1. About These Terms

These terms apply to your use of this website and to the visits described on it. The website is operated by Alverika, a functional medicine practice based in Mendham, New Jersey. By using the site you accept these terms.

Maria Korogodsky, PA-C, Physician Assistant, provides the care described on this website. Licensed Physician Assistant in New Jersey, New York and Florida. Care is available only to patients located in New Jersey, New York and Florida at the time of the visit.

2. Services

Alverika offers whole-person evaluation, conventional lab tests where appropriate, and lifestyle guidance on diet, sleep, exercise, and stress. Most visits take place online, and in-person visits are available by request in Mendham, New Jersey.

3. The Discovery Call

The free 30-minute discovery call is an introductory conversation. It does not include an examination, diagnosis, lab orders or treatment. Those are part of paid visits.

THE PATIENT AND ANY OTHER PERSON RESPONSIBLE FOR PAYMENT HAS A RIGHT TO REFUSE TO PAY, CANCEL PAYMENT, OR BE REIMBURSED FOR PAYMENT FOR ANY OTHER SERVICE, EXAMINATION, OR TREATMENT THAT IS PERFORMED AS A RESULT OF AND WITHIN 72 HOURS OF RESPONDING TO THE ADVERTISEMENT FOR THE FREE, DISCOUNTED FEE, OR REDUCED FEE SERVICE, EXAMINATION, OR TREATMENT.

4. Visit Fees and Your Good Faith Estimate

Visits are cash-based and are not billed to insurance.

You have the right to receive a Good Faith Estimate of expected charges.

You have the right to receive a "Good Faith Estimate" explaining how much your health care will cost. Ask for yours when you book a visit.

Lab tests are not included in visit fees. Labs are billed separately by the laboratory, and some may be billed to your insurance.

5. Payments

This website does not collect payments. Payments are handled outside the website.

6. Appointments and Access

You are responsible for being located in New Jersey, New York or Florida at the time of a visit and for having a device and a connection that allow an online visit. Please tell us if you need to cancel or move a visit.

7. Privacy

How personal information is handled is described on the privacy policy page of this website. The contact form is not a place for medical details. Share those during a visit.

8. Intellectual Property

The text, design, and images on this website belong to Alverika or are used with permission. You may view the site for your own use. You may not copy or reuse its content for another purpose without written permission.

9. Disclaimers

The information on this website is general and is not medical advice. It does not replace a visit with a health care provider, and it does not create a patient relationship by itself. No outcome or timeline is promised.

10. Limitation of Liability

To the extent the law allows, Alverika is not liable for indirect or consequential damages, or for delays beyond its reasonable control. Liability for any claim is limited to the amount paid for the service that gave rise to the claim.
